Medication Therapy Management (MTM) provides a broad range of health care services by the pharmacist. Our Pharmacist are well trained and certified MTM providers. It improves the therapeutic outcome for individual patients by managing their medications.
It is a five step process:
1. Medication therapy review (MTR)
2. Personal medication record (PMR)
3. Medication-related action plan (MAP)
4. Intervention and/or referral
5. Documentation and follow-up
YES. Most of the insurance covers MTM service. We accept most of the insurance plans including medicaid and medicare part D.
YES. Schedule an appointment prior to do MTM.
The Pharmacist will need to see all the medications(filled at any pharmacy), OTCs, herbals, blood sugar testing supplies.
Our pharmacist may also want to see your previous blood glucose, blood pressure, cholesterol readings, etc.
